Heart is Home Cooperative Care offers a range of personal care services that makes it easier for clients to be healthy, safe and happy in their home. Heart is Home does not provide clinical services such as nursing, physical therapy, hospice or other such services, but will work in coordination with agencies that may be providing these services to our clients to ensure positive experiences and outcomes for them.
Our minimum is 3 hours per visit.

Caregivers may not give medications to clients. Clients must have medications organized in pill boxes according to physician’s orders.

If client declines or is unable to take his/her medications, this should be documented in the PCA/Homemaker Visit Record.
Caregivers will provide healthy and well-balanced meals and snacks for the client. Caregivers will prepare food in the client’s home, using USDA MyPlate guidelines and proper food safety protocols. Client food preferences and dietary needs will be met. Caregivers will prepare meals according to the client’s preferred schedule.
This include daily services such as washing dishes used by the client and/or caregiver, loading and emptying dishwasher, sweeping, tidying up client area of activity, making the bed, light cleaning in the bathroom, taking out trash. Weekly services include vacuuming, watering house plants, recycling collection, mopping floors, light cleaning and organizing of refrigerator, changing bedding. Caregiver, utilizing the client’s washer and dryer we will launder clothing, bedding and towels at least twice a week (and more frequently when linens become soiled).
At an additional cost: We offer basic pet care may be provided by the Caregiver, including feeding and letting pets in and out of the home into client’s yard. If additional pet care services are required beyond those described here, the client and/or family are encouraged to arrange services with outside contractors.
Caregivers may transport clients to medical appointments, to the grocery store and on other outings. Arrangements for transportation must be arranged through the Caregiver Manager. While every effort will be made to accommodate requests for transportation assistance, a minimum of five days prior to date requested is appreciated to coordinate transportation.
Clients are asked to fit appointments and other transportation needs to the Caregiver’s scheduled regular times of service as it is difficult to meet transportation needs outside of a Caregiver’s regular schedule.
Services include activities such as games, puzzles, viewing photographs, reading, walking, listening to music, writing letters, crafts, light gardening and other activities. Our Caregivers commit to spending quality, interactive time with clients that reduces television time, encourages movement and brain activity and keeps sleep time in a healthy balance. Activities are recorded in the Aide Visit Record.
